Emergency Evacuation - R Apr 2025
A walk through of the playground during the visit indicated the following: -There is a small child sized bench adjacent to the picnic table that has chipped and peeling paint that is in need if repair. -There is an adult sized bench across from the picnic table next to two potted plants that has chipped and peeling paint that is in need of repair. *The outdoor play area must be free from hazards including but not limited to chipping, peeling or flaking paint.
A review of the program's emergency evacuation/fire drill log on the day of the visit indicated that there was no drill conducted for the months of October 2024 through January 2025. The program must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

Current consent for administered medications - R Apr 2025
A review of the Children's Record Checklist indicated that there were 17 children listed as either having an outdated physical exam (2022) or no date indicated on the checklist of a physical exam being recorded.
A review of three Children's files indicated the following: -One child's record did not contain evidence of a Medication Consent Form for a prescribed medication.
A review of the program's Staff Record Checklist along with a review of three staff records via email following the visit indicates that 3 staff do not have evidence of current physicals.

The program's two administrators were not onsite at the time of the visit (2:45pm). However, an educator onsite stated they were Director certified and called one of the administrators and that administrator came back to the program to meet briefly with the Licensor. The Licensor requested documents be sent for review. An email was sent by Licensor following the visit on 7/9/24 outlining this request.
This follow-up visit inspection recorded no violations or advisories.

Individual Health Care Plan (IHCP) - R Apr 2025
Upon a review of two Children's records at the program who were diagnosed with a medical condition requiring an IHCP, the following was identified: -One child did not have current and complete Medication Consent Forms for either a prescribed (REDACTED). -One child did not have a Medication Consent Form for a prescribed (REDACTED). This means the program did not have parental consent to administer medication in the event of a potential medical emergency for two children.
A review of medication stored on site during the visit indicated the following: -A child's prescribed (REDACTED) was not in its original box with affixed prescription label.
A review of two children's Individual Health Care Plans indicated the following: -One child diagnosed with (REDACTED) did not have a current IHCP on file.
Health Care Consultant Agreement
The Building Inspection Certificate expired 1/30/2022. The Fire Inspection Certificate available for review was conducted on 12/16/2020. The Indoor and Outdoor IPM was not current at the time of the visit.
Upon a review of three Children's records at the program who were prescribed medications, two records did not have current and complete Medication Consent Forms. This means in the event of an emergency, the program did not have parental consent to administer potentially life saving medication (REDACTED) to two individual children.
The Health Care Consultant Agreement was not current at the time of the visit. The program did update and submit the HCC agreement on 4/28/22.

Upon a review of Individual Health Care Plans of children at the program, the following was found: -Two children did not have complete and current Individual Health Care Plans.

Building Inspection and Fire Inspection - R Apr 2025
In the program's document library in LEAD, the following inspections are not current: -The Building Inspection Certificate expired on 10/18/19. -The Fire Inspection was last completed on 10/18/18. This non-compliance was cited on 10/11/18 as follows: The licensee did not have an annual inspection of the building as required. The licensee did not have an annual inspection of fire codes. Please remit current building and fire inspections.
