At the time of the visit, it was observed that one of the educators in the preschool classroom walked into the kitchenet area, which is in a separate space from the classroom, behind a door. It was identified that the infants bottles are kept in the fridge within the kitchen area, and the educator was getting a bottle for the infant classroom. This left an Assistant Teacher alone with the group of sixteen preschool students. All licensees and educators must exercise appropriate supervision of the children in their care in order to ensure their health and safety at all times. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities.

At the time of the visit, it was observed that the amount of woodchips on the Preschool Playground, that surrounded the climbing structure, resulted in the metal fence being under four feet high. At the time of the visit, the height of the fence by the climbing structure was measuring at 3ft 1inch. It was also observed that one of the gates within the preschool playground did not latch and was open. Another gate, within the garden area, did not latch properly and could be opened very easily. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

At the time of the visit, it was observed that on the Preschool playground there was a metal fence that abutted the white privacy fence. The two fences were not attached, which left a gap greater than 3.5 inches and less than 9 inches, creating an entrapment hazard. The gap varied in size from the ground surfacing to the top of the metal fence. All play equipment, fences and structures must be free of entrapment hazards.

Overgrown grass, and weeds within the infant, toddler and preschool playground. A bucket on the toddler playground had a large crack on the lid. The woodchips on the toddler and preschool playground were not at adequate depth - the toddler playground had woodchips about 1/2 to 1 inches in depth and the preschool playground had woodchips at about 1 to1 1/2 inches in depth. On the bench leading out of the preschool classroom, a sprinkler with a sharp bottom was accessible to children. There was a metal rake accessible to children within the infant playground/ near a walking path to the PS playground. The school had broken siding; the crack in the siding was accessible to the students walking by. The lock on the PS gate was broken and no longer screwed into the fence. A metal pole was in use to block off a slide on the PS playground; this is not a safe means to block off an area that is accessible to students. Three bikes on the PS playground were found to have rust. The fence on the PS playground had rust.
