The first aid kit was missing the emergency cards for the two children who are currently enrolled in the program.

The records were reviewed for the two enrolled children. Signatures were missing for the Acknowledgment of the Parent Handbook in both enrollment packets. In one of the enrollment packets, the signature was also missing for the Parental Visit Notice. One record was missing proof of the child’s current physical and immunizations.
