At the time of the visit, in a review of three IHCPs, the information was not fully complete and/or current. Two students had expired IHCPs. One student was missing an IHCP for their diagnosed heath condition. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner.

Emergency medications readily available
At the time of the visit, the hallway used to access the back door, that leads to the playground area was not safely maintained. Within the hallway the following was observed: the flooring was cracked, there was a mop, two large trash barrels and a ladder being stored in the hallway. Directly outside the entrance of the hallway (within the cafeteria space) there was two large mops and dustpans being stored. The licensee must ensure that space is arranged to provide clear pathways for movement from one area to another and to allow visual supervision by educators.

At the time of the visit, the person identified as the Site Coordinator did not have a staff file on site. The qualifications of the Site Coordinator were unable to be determined by EEC. The licensee must ensure that the program is soundly administered by qualified persons designated with specific administrative and program responsibilities.

At the time of the visit, in a review of three students files the information was not fully complete. One student was missing documentation of First Aid/Emergency Medical Consent & Release. Another student had expired Off Site Consent. Children's records must have all required applicable documentation according to the Children's Record Checklist.
At the time of the visit, in a review of four educators files, the information was not current and/or available. Two educators did not have a file on site to review. Within the two files that were on site, both were missing documentation of Program Orientation and current staff observations. Both had expired PQRs. One file was missing documentation of completing CPR, First Aid and was missing documentation of Professional Development Hours. The Program must maintain a personnel record for each staff member.
At the time of the visit, Group One had twenty-two students present. The attendance reflected twenty-one students. At the time of the visit the educators within Group One were able to identify what student was not signed in and the attendance was updated to reflect all students present. Daily attendance records indicating each child's attendance, including arrival and departure times, must be maintained by the program.
At the time of the visit, it was observed that the Site Coordinator was working with the Elementary School's Office to determine if certain students were going to be absent from the after school program. They left the cafeteria to communicate with the office. It was also observed that educators entered the back storage room when they were responsible for supervision. Educators leaving the cafeteria left the group out of ratio. Educators must be positioned to maximize their ability to see and/or hear children in their care. Educators must exercise appropriate supervision of the children in their care
At the time of the visit and in a review of the programs document library, the Building Inspection and Fire Inspection were expired. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
At the time of the visit, in a review of the Emergency Evacuation Log, the information was not current. The program was missing documentation of completing January 2025, February 2025, March 2025 and April 2025 evacuation drills. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, in a review of three IHCPs and the corresponding medications, the information was not current. Two students had expired medications. Medications must be appropriately stored, labeled, not expired and correctly administered.

At the time of the visit, in a review of three IHCPs and the corresponding medical documentation, the information was not current and/or fully complete. Two students had medication consent forms that were not the EEC Medication Consent form. Both students were missing required medical information on the provided medication consent documentation. Another student had two prescribed medications, one medication consent was expired and the second medication consent was not on site. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.

The ceiling within the girls bathroom had water stains and the sinks had areas of rust.

A review of background record checks indicated that one educator did not have evidence of a completed BRC and another educator had an expired BRC.

There was not a posting, visible to parents, educators, and visitors, listing children's medication/allergies.
There was not an Individual Health Care Plan for a child receiving medication at the program for ADHD; and for a child with an Epi pen/Benadryl/Albuterol.

Indoor Space
A review of the children's records indicated the program completed written progress reports. However, the program did not provide a copy of the reports to the parents and they did not offer parent conferences.
The following was observed: GIRLS BATHROOM -There were 11 water stained ceiling tiles. BOYS BATHROOM -There were eight water stained ceiling tiles. -Four of the heating vents located above the urinal toilets had rust on them.
The Building Inspection Certificate expired on 8/30/2021. Please submit an updated inspection to the Department.
