The records were reviewed for the enrolled children. Four children were missing the Annual Update form and proof of current physical, immunizations, and lead screening. One child was missing the signature for the Receipt of the Parent Handbook.

The Educator has 22 professional development training hours as of the day of the visit. Discussed she needs 8 more hours before her license expires. She does have 10 hours of diverse learners training hours. Please send proof of 8 additional training hours.
Records were reviewed for the five enrolled children. Four children were missing proof of current medical documentation in their files. The Educator was able to find some of the missing documentation that was emailed to her. Two of the parents sent the medical documentation during the visit.

Progress reports were current for all enrolled children. One child needed a progress report; the other four were current.
The vaccinations were current for one dog. The other two dogs and the one cat need proof of current vaccination records. Please send proof of vaccinations.
There were 2 outlets in the dining room that were uncovered and accessible to the children.
There was a drawer in the bathroom that had scissors, an electric trimmer, and grooming products accessible to the children. The trash in the bathroom was uncovered and accessible to the children. The child safety latches for the cabinet under the kitchen sink were not working, allowing children access to cleaning products, etc.) There were 3 drawers in the kitchen where the safety latches were not working, allowing children access to sharp objects (vegetable peeler, pizza cutter, scissors, etc.)
There were outlets in the living room, dining room, and basement playroom that were uncovered and accessible to the children.

There was one common towel for drying hands. Discussed there must be a towel that is individual to each child or a disposable paper towel.
The evacuation drills were not logged on the evacuation drill log since October 2022.
