1. At approximately 2:56pm, the group size exceeded the maximum of 26 with a total of 56 children. 2. At the time of the visit, staff to child ratio were fifty-six children, three staff and a volunteer, which exceeded the 1:13 ratio. 2. At the time of the visit, two staff and a volunteer reported not being aware of the total number of children present in the cafeteria.

A review of four staff records reflected the following. 1. Documentation of a program orientation and staff information forms were not available for review for three staff. 2. Documentation of {REDACTED} and a physical was not available for review for a staff member. 3. Verification of School Age Work Experience was not available for review for a designated Group Leader.
The staff schedule posted on site was outdated, (dating 9/2021). The staff schedule did not reflect the staff present on site.
Fire Inspection submitted in LEAD and posted on site expired 8/27/2022. The Building Inspection submitted in LEAD reflects expiration date of 9/1/2021.
Documentation of a practice fire drill for an April 2022, May 2022, June 2022, September 2022 and October 2022 were not available for review.
