At the time of the visit EEC observed the following: a) Three Individual Health Care Plans for children with a diagnosis of a chronic medical condition were not available for review. b) One Individual Health Care Plans for a child with a diagnosis of a chronic medical condition was expired. c) Educator was not knowledgeable about Individual Health Care Plans.
At the time of the visit EEC observed the following. a) One emergency medication was not available for review. b) Two medications were not in the containers in which they were originally dispensed and with their original labels affixed.

At the time of the visit EEC observed the following: a) Two Epinephrine Pens were expired. b) One albuterol had an expired prescription.
The staff records checklist reviewed during the visit indicated that some required information was missing and outdated.
This follow-up visit inspection recorded no violations or advisories.
At the time of the visit the program did not have the sufficient number of educators necessary to maintain staff child ratios.

1. [REDACTED] Individual Health Care Plans were incomplete and did not contain all of the required sections as per EEC regulations. 2. [REDACTED] child’s IHCP stated n/a on most of the form although they did have a separate [REDACTED] consent form for an [REDACTED] and there was an [REDACTED] on site. This same form was also not signed by the doctor.

First aid certification
The program exceeded ratios of 1 Educator to 13 children. Upon arrival to the program and two other separate times during the visit, the [REDACTED] was with a group of 18-22 children in the cafeteria. The second staff member was with the remaining children who were washing their hands in the bathroom.

At the time of the visit three staff records did not have documentation of a current first aid training.
[REDACTED] Educator who has been employed at the program for [REDACTED] did not have documentation on file of first aid certification.
Clorox wipes were on the table and in an unlocked cabinet that was accessible to children.
