Emergency Evacuation
During the visit, it was noted that in at least one classroom the privacy protected allergy list was expired.
During the visit it was noted that a couple rooms did not have the emergency evacuation procedures/diagrams posted.
During the visit, it was reported that the program has not practiced regular emergency evacuation drills as required.

Infection Control/Hand Washing
In the toddler room there are several areas of the floor tile that have lifted and could lead to a tripping hazard. The flooring tile in the preschool room is lifted leading into the bathroom.
During the observations it was noted that a mop and bucket was located in the children's bathroom next to the toilet. The mop head was stored in a manner in which children could easily touch.
In several classrooms there is evidence of peeling paint. In two classrooms the licensor observed small holes in the walls.

Building Inspection and Fire Inspection - R Apr 2025
During the visit it was noted that both the Fire and Building inspections are expired.
During an observation of the playground it was noted that there is not sufficient impact absorbing materials under the play scape. A little tykes platform slide that is higher than 18 inches is located on a grassy area that does not have impact absorbing materials underneath.

Toddler room 1 had limited materials available to children. Materials should not be limited for children based on a child's need for developmental intervention. Appropriate materials and teacher interaction should be used as tool to promote self regulation for children.

During the classroom observations it was noted that one toddler room changing table surface had several large black stains. The plastic changing table surface was also torn potentially leading to cross contamination.

Individual Health Care Plan (IHCP) - R Apr 2025
Three new employee did not have current First Aid training within six months of hire date.
Toddlers were observed playing with "Orbeez" in the water table. This product is a sensory toy for children ages 5 and up. The product contains a warning "Choking Hazard: Small parts Not for children under 3 years"
Overgrown shrubs protruding through fence near toddler play structure should be trimmed. Remove broken plastic toys on preschool playground. Repair broken lattice and climber located in the side yard. Two large storage deck boxes were unlocked and within the reach of the children.
One child's inhaler was not in the original container. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturers packaging.
The following were within reach of the children in the bathrooms: bleach bottle, plastic bags, plunger and creams.
The three medication plans that were viewed during the visit, did not contain all of the information required for IHCP.
