During a review of ten children's records, progress reports were not found in files. The [redacted] reported that progress reports were not conducted once a year for school age children.
During a review of children's records, it was found that at least four children were missing enrollment paperwork. The files contained blank forms.
It was learned that at least four children were missing consent for emergency medications.
During a review of staff records, it was found that three educators were missing BRC suitability letters, and one educator did not have current first aid certification or an updated PQ Registry certificate.

During a staff file review, there was no evidence of a completed BRC, and the suitable letter was missing from two staff files.
During a review of children’s records, it was found that one child was missing an IHCP and an EEC medication consent form was missing a doctor's signature and one child was missing the medication consent form and the parent needs to sign the IHCP.

It was learned that two educators on the premises on the day of the visit did not have a suitable BRC.
The staff records checklist was not available at the program for review.
A program administrator meeting the qualifications was not on the premises during the hours of program operation. Two assistant group leaders were left alone with 14 children.
During a review of children's emergency medications, it was found that two inhalers were expired.
Educators were not knowledgeable about Individual Health Care Plans for children who were attending from another site during school vacation week.
Upon arrival, it was observed that staff child ratio was not met, and there was one educator with 18 children in the auditorium.
