At the time of the visit four child files were reviewed and the following was found: Child A was missing a developmental history and transportation plan. Child B needed all annual documents updated and had an expired physical. Child C had no physical and needed annual documents updated. Child D needed a current physical and updated annual paperwork.
A copy of the current staff schedule was not able to be located for review at the time of the visit.
The program did not have a current fire inspection available for review at the time of the visit.
The paint on the baseboards in Infant C classroom was peeling. This was accessible to the children.
Two children in the "Multi D" preschool classroom did not have completed developmental placements. They were under the age of 2 years 9 months but did not have all the proper documentation to support this placement.
At the time of the visit the Toddler A classroom was not able to locate their emergency bag, that contained a child's emergency medication.
At the time of the visit the Toddler D classroom did not have a sink in close proximity to the diaper changing station. There was a diaper changer in the classroom however staff and children had to walk through another classroom to get to a sink to wash their hands.

At the time of the visit not all staff had current BRCs and some were in need of renewal.
At the time of the visit preschool napping blankets were all stored in a pile, touching one another. Bedding should be stored in a manner in which they do not touch one another.
Over the past year evacuation drills have not been logged consistently. Drills were missing for May, June, July and September of 2023.

In INFANT C, two children were observed to be holding and drinking from their own bottles without being held by an Educator. One child was leaning against a Boppy pillow and the other was moving about the rug on the floor.
The following was observed and accessible to children in classroom K PREP: -A broom leaning against the wall in the bathroom. -A rusty metal plate behind the toilet where the pipe meets the wall in the bathroom. -an unlocked storage closet with unsecured stacked water bottles and boxes. The following was observed and accessible to children in classroom PRE K: -A Swiffer broom and mop leaning against the wall in the bathroom. -A rusty metal plate behind the toilet where the pipe meets the wall in the bathroom. -The sprinkler room door unlocked. The following was observed and accessible to children in classroom TODDLER A: -Trash can uncovered. -Dirty diaper on the floor in front of the changing table. -A broom leaning against the trash. -The swinging door that separates the Toddler A from Toddler D has chipping paint/plastic along edges. -The unlocked bathroom used as storage for cleaning devices such as a Swiffer mop and broom. The following was observed and accessible to children in classroom TODDLER D: -Rest cots stacked high and tippable. -Rust on the wall vent at the far end of the room. -Trippable green floor rug in front of the toddler sinks. -Sticky residue on the top of the low bookshelf. -The unlocked bathroom used as storage for cleaning devices such as a Swiffer mop, dustpan, and spray cleaning bottle. The following was observed and accessible to children in classroom Infant C: -The changing table contained 4 spray cleaners. -Low white shelf on left of room unanchored. The following was observed and accessible to children in classroom Infant A: -Crumbs observed in two cribs. -Door tracks under the sink were dusty and contained crumbs. The following was observed and accessible to children in Discovery Preschool: -Sticky residue on top of two bookshelves. -Tape no longer secure around the toy sink and collecting crumbs. -Dust on the wall vent. -A visibly dirty dust pan hanging low in the bathroom. -Door tracks under the changing table were dusty and contained crumbs. The following was observed and accessible to children in TODDLER C: -A large amount of tape reside on the right side wall where a sticker chart used to hang. The following was observed and accessible to children in the GROSS MOTOR SPACE outside of Toddler C: -Two tippable bookshelves and a basketball hoop. -Chipping paint along the blue painted walls. The following was observed and accessible to children in PRESCHOOL: -Lego tables containing dust and crumbs. -Tippable bookshelf in the hallway space outside of the room.

The following hazards were observed in the unlocked kitchen and are accessible to children: a low unlocked cabinet full of spray cleaning products and a stove.
The following was observed outdoors: -Rust chipping on the bottom metal plates connected the canopies to the cement in the toddler playground space and in the back preschool playground space. -Rust chipping on two of the canopy poles in the toddler playground space.
A review of the Evacuation Drill Log indicated drills were not conducted in July, August or September 2022.

One educator stated that a child's (REDACTED) was located in the office. A review of the child's file indicated the child did not have (REDACTED). A review of a child's file indicated that he has an (REDACTED). There was not an Individual Health Care Plan (IHCP) on record for the child. His current physical does not indicate the (REDACTED). A review of a child's file indicated that his current physical examination says he has (REDACTED), with a (REDACTED). There is not an IHCP on record for the child, and no medication is at the program for him. A review of a child's file indicated he has (REDACTED); the IHCP dated (REDACTED) is not complete. A review of a child's file indicated that an IHCP for a child with (REDACTED) is not available.
In Infant A torn books were accessible to children; the labeled dresser is dirty; the large dresser (used for extra storage) does not have a lock on one drawer which is accessible to children; the diaper changing table tracks were dirty; the sink frame was cracked. In Toddler A a storage cubby is not secured. In Toddler C a wooden play sink is missing the sink, the diapering sink is dirty/rusty/with a gel type substance (glue) on it; torn books were accessible to children. One bathroom had excess tape residue on the wall. In the Learning Center the blue vinyl couch had frayed areas; one cubby shelf was not secure; one black desk table has sharp corners and was not secured. In Preschool the lego table is dirty; a plunger is accessible to children; the diaper changing area is dirty; the wooden baseboard is chipped.
A closet in the Learning Center area door was open, a teacher purse was accessible to children.
Staff stated that one child's was given an (REDACTED) "last week"; however there was not documentation of the administration of medication available.
