A child in the CH1 classroom left the room to get something from her cubby. Staff allowed the child to leave alone and close the door with no direct supervision.

At the time of the visit Children in CH1 were using a loft in an unsafe manner. They were not being supervised while climbing up a ladder that had no railings and were standing up once at the top of the loft and the barrier was not tall enough to prevent a child from falling over the side.
At the time of the visit there was a cup of coffee accessible to the children and a staff purse was accessible to children in the CH 1 room. Travel mugs were on a counter and within reach of the children. There were sharp objects in a drawer in the toddler room accessible to the children.
At the time of the visit there was missing information from individual health care plans.
At the time of the visit there was expired medication in the CH1 room and the toddler room.
At the time of the visit a child who was playing in the water table did not wash their hands after they were done.
At the time of the visit the CH1 room had cereal that did not include an expiration date. The CH2 room had expired pretzels.
