It was observed that bedding was not stored in a safe and sanitary manner. Sheets were on cots in two classrooms and touching one another.
During an inspection of the outdoor space, it was found that the climber and metal vehicle on the preschool/school age playground has rust. 7.07(13) Safety Requirements for Equipment, Materials and Furnishings. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children.
It was observed that the slide exits exceeded 11 inches from the protective surface on two slides in the preschool and school-age playground.

Toxic Substances
It was observed that the attendance records were not accurate in two classrooms, and two children were not recorded on the attendance form.
Hazards were found in the program. Hand sanitizer and a pocketbook were accessible to children in the Preschool Room (Starfish Room) and an electric fan and a pocketbook were accessible to children in the in the basement (Stingray Room).

It was observed that the educator did not follow the proper hand washing procedures and the educator did not wash their hands or a child's hands after changing a diaper.
Emergency medications readily available
During an inspection of the playgrounds, it was found that the depth of the impact absorbing material (wood chips) did not meet the required 9 inches, a slide in the toddler playground was located in an area without a fall zone, and the slats on the stairs and ramp were a head entrapment.

During an inspection of the indoor space, it was observed that the windows did not have window guards or screens. All windows used for ventilation must include screens in good repair. Windows and glass doors must be constructed, adapted, or adjusted through the use of window guards or other means to prevent injury to children.
