Current consent for administered medications
During licensor observations a child was screaming, hitting, throwing things and taring up papers. Several educators tried to calm the child down by talking with them. At one point the child left the licensed space and was alone in the vestibule. The educators allowed the child to be alone in the vestibule out of sight from educators because the child was calm in there and liked it. The vestibule is about four feet by five feet and has two doors that open to the outside facing Jackson street.

One child with an allergy to [redacted] did not have their [redacted] on site, the Administrator reported the parent never brought one in, the child has been enrolled since school started.
A child with an [redacted] for an allergy to [redacted] did not have a parent signature on the medication consent form. The medication consent form was missing information. The Prescription label for the [redacted] was missing.
This investigation inspection recorded no violations or advisories.

The building and fire inspection were expired. The program had a letter stating the school failed the inspection, and needed to make corrections then schedule an appointment for a reinspection.

The staff records checklist is not kept on site. The site coordinator reported they are kept in the HR department.
Educators were not attentive to the children’s needs. The educators were all positioned in the front of cafeteria taking attendance, while children were wondering off or running around. Children were not given direction or offered transition activities while they waited for attendance to be taken.
The staff schedule was not posted. I was not able to verify staff qualifications or First Aide and CPR certifications.
No independent time offered during the visit, Half of the program went out to the playground while the other half had snack. No materials or activities were observed.

A child who had Benadryl did not have a prescription order or medication consent form.
A epi-pen was locked in a medication box, A child with a Albuterol inhaler written into his IHCP did not have the inhaler in the program.

Educators were knowledgeable about children's IHCP. But the IHCP reviewed did not have current children's needs listed. A child had a IHCP that called for a Epi-Pen. The program was aware that the child only needs Benadryl now and no Epi-Pen. But the IHCP did not reflect that.

Children’s records had required documents. Except IHCP, injury reports and progress reports. Progress reports were not available for review.
