A sampling of children's files was reviewed. One child's file did not have a current lead test.
There was a large amount of sand on the poured rubber surface on the playground. The sand made the surface slippery which may be hazardous to children.

There were several area out at the playground where there was sand (from the sandbox) on the rubber matting. The sand makes the rubber matting slippery and a potential hazard to children.
One child's Individual Health Care Plan (IHCP) included a medication dosage of 7.5 ML. The medication consent form included a medication dosage of 5 ML. This information is conflicting. The doctor must provide documentation of the correct dosage to be administered.
Review of monthly staff meeting documentation did not include at least 2 hours of staff meeting time for all staff for the past year.

Three children's current enrollment forms did not include their arrival and departure transportation plans.
Review of the practice evacuation drill log for the past year included the followng: 1) The program has been practicing evacation drills monthly with both classrooms. However, several drills were not conducted as an entire program; classrooms were evacuating the building at different times. Evacuations drills should be conducted with all groups of children/staff. 2) Documentation included that the T/PS classroom did not conduct any drills after 10:30AM; the PS classroom did not conduct any drills after 3:30 PM. Please ensure that drills are held during different times of the program day,
