In the Pre K classroom, three children were not in direct visual supervision of the educators. Educators must be positioned to maximize their ability to supervise children in their care.
In the Pre K Classroom, electrical cords were easily accessible to children.
The following was noted in the 3 Day Classroom and Pre K classroom: 1. A shelf in the Pre K Classroom had a cup of hot liquid easily accessible to children. 2. A cup of hot liquid was on top of the children's cubbies. 2. The latch to the boiler room door in the Pre K classroom was unlatched.
The location of the first aid bag kit and health care policy was not posted.

This investigation inspection recorded no violations or advisories.
Of the 6 children requiring medications/IHCPs, none of them had the IHCP found with the medication; 2 IHCPs called for an additional medication that was not on site; one child had IHCP with no parent signature; 2 children had IHCPs with no doctor signatures; one child had an additional medical condition listed (REDACTED) and had medication on site but no IHCP or medication consent; 1 child's medication missing original packaging.

Staff monitor environment for hazards-Hazardous items not accessible to children
Three staff files were reviewed and the following was noted: 1. One staff's orientation checklist form was incomplete. 2. One staff's information form was missing a director's signature and date. 3.One staff's information form was missing the date of hire. 4. One staff's reference check was not documented effectively.
The staff meeting documentation did not show evidence of a staff meeting occurring in month of September of 2018 and in 2019.
The children's records checklist and three children's records were reviewed and the following was noted: 1. Three records were missing the date of admission and anticipated days and time of attendance. 2. One child's record was missing parent authorization and consent on the child's First Aid and Emergency Consent form. 3. One record did not have documentation of lead results. 4. The children's record checklist was incomplete and inaccurate and did not reflect the content of some children's files.
In the 3 Day Classroom, an electric fan was easily accessible to children.
A garbage barrel used for trash was missing a lid.
Glassware was utilized in the 3 Day Classroom and Pre K Classroom posing a potential injury hazard to children.
