A sampling of staff files was reviewed. 1) Two staff files were missing documentation of education to qualify them for their current assigned position. 2) One staff file was missing the date and documentation of the interview. 3) Two staff files were missing a current physical exam. 4) One staff file was missing 2 doses of MMR or proof of immunity.
Two exits in the gym did not include posted written evacuation plans including where to meet upon evacuation from the building.

One Medication Consent Form included a dosage of 7.5ML. The medication provided to the program was in solid form (MG). The program could not determine the conversion and therefore did not know the correct dosage to administer to the child should they show signs of a minor allergic reaction.
The written evacuation plan was not posted at the exit in Room 119.
The Allergy/Medical Condition List did not specifically name the emergency medication available for each child.

A plastic bottle of disinfectant wipes was observed on a cafeteria table accessible to the children.
1) One child's emergency medication container had an expiration date of 4/21 even though the date on the medication box was current. 2) One child had 2 emergency medications. A Medication Consent Form was not completed for each medication and signed and dated by the parent.
The Fire Drill log was reviewed. A practice drill was not conducted in 11/21. Documentation for drills did not include the route used.
An exit in the lunchroom/cafeteria did not include written evacuation procedures posted at the exit.

1) Previous attendance sheets were reviewed. One child’s time of departure was not included on 12/4/19. 2) On the day of the visit, a group of children were observed in the gym in karate class. The PA/SC reported that the children are supervised by the program staff during karate. Staff did not have a method of knowing which children were in that class at all times.
One child's Authorization for Medication Forms did not include possible side effects of the medications.
Writen evacation plans including the specific place to meet upon evacuation were not posted at each exit in the gym.
