The licensor observed 6 of the children records. The following was observed: 1.One of the children physical did not have information if the childld was tested for lead. 2.One of the children did not have an updated physical (last physical was 5/3/23). 3.One of the children records did not specify on the general permission (basic transport) where the child is allowed to go (see page 6)

The licensor reviewed 6 of the progress reports. The following were observed: 1. Three of the children did not have updated progress reports. The last time reports were completed were: 6/7/23, 10/2023, 10/12/23 2. One of the children is not due for progress reports yet. 3. Two of the progress reports were complete.
The educator did not have an emergency plan.
The licensor observed that the changing surface is not protected with a covering that an adequate size to prevent the child from coming in contact with the changing surface.
Progress Reports
The licensor reviewed 5 of the children's records. The educator still needed to complete the progress reports for the five children.
During the visit, Licensor observed two different structures (castle with red slide attached=28 inches; single orange slide=20 inches). Both of the structures measured more than 18 inches in height, and the educator did not have a Fallzone as required. Note: During the visit, Licensor observed a swing set, and a large structure with three different slides attached which have been previously approved.

During the visit, Licensor reviewed the 7 progress reports of the children that were present, the following was observed: 1) four children progress reports were not up to date. 2) two children the progress reports did not have the completion date.

Emergency Preparedness Plans
Licensor reviewed the Monthly Evacuation Log and observed the following: 1) The educator is not alternating Emergency Exits as required. Note: Last fire drill documented 7/8/2021.
Licensor reviewed the children's records checklist submitted by educator on 7/18/21, and observed the following: 1) a child's physical was not up to date.
Licensor review the Emergency plan submitted by the educator, and observed the following were missing: 1) Lock down procedures 2) Missing Child
