The educator did not have the individual health care plan for two children who have a chronic condition.
The licensor reviewed 4 of the children’s records. The following were observed: 1.One of the children records did not specify on the general permission where the child can go when off the premises. 2.One of the children records did not: A. have the educator’s name on the permission to transport to medical facility and receive emergency medical treatment. (Page 6). B.The child’s name on the general permission (basic transportation) is not listed. 3.One of the children records did not have the child’s correct date of birth listed. 4.One of the children records is not complete when the child has a health condition and it did not list the child's medication.

The licensor observed that the educator did not have a current consent to administer medication for one child.

The licensor reviewed 5 of the children's records. The following were observed: 1. One of the children's records was not accurate when the child’s medication was not listed. The child’s medication is listed on the individual health care plan. 2. One of the children's records is not accurate when the child’s individual health care plan expired. The plan expired 1/30/24 3. One of the children's records is not accurate when the information about allergies says “ may have asthma ”, and the annual update form states none is needed, meaning medication. The parent fixed this during pick-up time. 4. Two of the children's records are accurate.

During the visit, Licensor observed the educator had 25 children enrolled, however the educator only had 7 Emergency Cards/family contact information in the first aid kit, and all children must have Emergency Cards.

During the visit, Licensor observed the educator did not have practice Evacuation Drills documented for the months of March, April, and May 2022.
