The children's record review reflected the following: 1) A record contained no evidence of periodic progress report and current transportation form. 2) A second record contained no evidence of transportation form. 3) Two records contained no evidence of periodic progress report, current transportation form and annual lead screening.
The staff record review reflected the following: 1) A record contained no evidence of current physical. 2) Five records containing no evidence of ongoing bi-monthly observation for 2025 and no evidence of a current annual evaluation.

During the visit, two educators did not report a child's allergy when asked, despite the child being present and assigned to their classroom for the day. All educators must be aware of the contents and requirements of a child's individual health care plan.
