Emergency Evacuation
A review of emergency evacuation drills indicated that the program did not have evidence of completing evacuations for January, February, March, April, and May 2025. All programs must practice emergency evacuation drills monthly with all staff and children. They must be documented.

Staff orientation, training and meetings
A review of Staff Meetings for the program indicated the following: -The licensee did not have documentation of completing staff meetings for the months of October and December of 2023.

Outdoor Space - R Apr 2025
An observation of the playground indicated the following: -On the large structure there was rust in multiple areas, including the bolts and metal connecting pieces. During the visit, the Director shared that the program is in the process of creating a renovation plan for the outdoor playground space, as this was previously cited in Visit# 110585 in June of 2022.
A walk through of the facilities indicated the following: -The paper towel dispenser in the basement classroom had sharp and rusted edging.

The MINOTS and DOLPHINS Classrooms did not have accurate attendance. Although staff knew who was there by memory, the DOLPHINS had not marked one child when he entered the program and the MINOTS had not marked three children. Another child on the MINOTS attendance was already marked as having left although she had not. The Educator explained she marked the child leaving in two hours to remind herself.
Three staff records were reviewed. All three were missing bi-monthly observations and annual evaluations.

There were shelves on wheels that were tippable. In addition one tiered library shelf was secured with velcro.
Indoor Space - safe, clean, in good repair
A review of three Staff Records at the program indicated that one educator did not have evidence of an MMR.
-The program had a current Building Inspection that expires on 7/31/23. -The program did not have a current Fire Inspection. During the visit, the Director stated the Fire Inspector is scheduled to come out within the next week.
A walk through of the facilities indicated the following: -The basement (red) floor has a significant amount of chipped and peeling paint throughout the art room, downstairs classroom, and bathroom. -The blue cubby located next to the egress exit stairwell in the basement had chipped and peeling paint. -The kick plates in the stairs leading from the basement to the first floor had chipped and peeling paint.

Medication
Emergency medications readily available
Three child records were reviewed. One child was missing a progress report conducted in the last 6 months.
Flaking and chipping paint was observed on the white trim near the back side entrance.
Rust was observed on the tall play structure on the bolts and metal connecting pieces. Rust was also observed on two unused umbrella bases seen under the picnic table in the bicycle area.
The number for Poison Control was not included on the emergency posting.
The(REDACTED) for a child with a (REDACTED) allergy expired in December 2021.
The emergency medication is kept in a metal box on a shelf in the back of the Director’s office. While it is inaccessible to children, staff reported the medication does not travel outside with the child as required. All emergency medications must be readily available at all times.
