Records were reviewed for the 9 enrolled children. Two did not have enrollment packets; the Educator reported the program had re-opened the day before the visit after having been closed for the summer. She stated she had given the packets to the parents to review and two parents had not returned the packets yet. There were also some children who needed proof of current physicals and immunizations.
