A sampling of staff files was reviewed. 1) One staff file was missing a current physical exam. 2) Three staff files were missing documentation of bi-monthly observations.
There were 2 open boxes of food that were not stored in clean/covered containers.

The thermometer in the infant classroom read 44F. Food requiring refrigeration must be stored between 32F-40F.
In review of the Staff Meeting Log, it was determined that the log did not have consistent evidence that they met each month for a minimum of 2 hours. 1) The Staff Meeting log included 1.5 hours of training on 2/24. 2) Several entries in the Staff Meeting Log did not include the end time or the start and end time of meetings. EEC was unable to determine the length of the meetings.

A sampling of staff files was reviewed. One staff file did not include evidence of 2 MMRs or proof of immunity. The staff member had been employed for 6+ months and their file did not include current First Aid certification.
In the preschool classroom, there was an uncovered trash can that included food.
Emergency numbers posted did not include the name, address, and telephone number of the Health Care Consultant.
Fire Drill documentation did not include the route used. The program needs to ensure that they practice using alternative exits.
Two opened cereal boxes and an opened sleeve of crackers were found not stored in clean, covered containers.
One child's Individual Health Care Plan included that if a child still showed signs of an allergic reaction after being given one dose of medication, another dose of medication could be administered again after 5 minutes if needed. The parent only provided the program with one dose of medication.

In the Preschool Classroom, there was a lamp on a shelf. The cord to the lamp was loose and accessible to children.

In the toddler room, EEC observed 4 children sleeping in an area off of the main classroom. At this time, a staff member was organizing items in the cubby area and could not see the children while sleeping.
In the Infant Classroom, the refrigerator thermometer read 45F. Food requiring refrigeration must be stored between 32F-40F.
There was a plunger accessible to the children in a preschool classroom.
1) One child's medication that is to be administered by EMS expired 11/30/22.
One preschool child used the bathroom and went right to their mat and did not wash their hands.
One child's IHC Plan included a medication for a mild allergic reaction. The parent has not provided this medication to the program.
1) One child's Individual Health Care Plan (IHC) did not include staff who were trained in medication administration and who trained the staff in medication administration along with the parent signature/date. 2) One child's two IHC plans did not include the possible medication side effects, consequences of the medication is not administered, staff who were trained in medication administration and who trained the staff along with doctors signature and date. 3) two children's IHC plans included that a second dose of medication may be given in child continues to show signs of allergic reaction after a period of time. The program only had one dose of medication provided to them for both of these children.
