Three chidren's files were reviewed. 1) One child's enrollment form was incomplete. 2) One child's Individual Health Care plan did not include staff who were trained in administering the medicatin and who trained them; consequences if the medication is not given and possible side effects. 3) One child's file did not include current evidence of a [REDACTED] and results.
The licensor observed Room 2F. A diagram of escape routes was not posted conspicuously at the means of egress in this classroom which does not have direct access to the outdoors.
Practice evacuation drill log for the past year was reviewed. The time of day was not noted for each drill.
