The EEC Licensor noted stones under the playground structures and throughout the entire playground area. Regulation 7.07(16)(e) 7.07(16) Article Playground Safety. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. Pea gravel and wood chip nuggets must not be used in areas used by infants and toddlers.

At the time of the visit there was a toilet brush accessible to children in the preschool bathroom posing a potential hazard.

At the time of the visit three children with emergency medical conditions were reviewed and the following was found: one child's IHCP was expired.
At the time of the visit there were 13 preschoolers in attendance and only 12 were signed in. The director corrected the attendance during the visit.
