At the time of the visit sanitizing bottle were within reach of children in one of the classrooms.
At the time of the visit the following was observed. There was equipment being stored in one side of the toddler room that was in the path of the bathroom. Carpet in one classroom under the sensory table that was curling and was a tripping hazard. Plastic kitchen set tippable and not secure. Some shelves throughout the program. Cleaside cleaning wipes were in the hallway on the floor accessible to the children. Door to the changing table in one of the infant classrooms was broken and falling off. Children's couch in the upstairs preschool was peeling and in disrepair. Large wooden cabinet in the basement school age area is broken and plexiglass has come out and was a hazard. Broom and dust pan hung so that the bristles are touching the door to the fridge. Spicket to the play sink was coming off. Window fans were propped in the classrooms on without being in a window.
Equipment is clean, safe, secured, and poses no hazard.

At the time of the visit the bathroom in the school age area had visible dust on the kickboard molding and the stools were in need of a cleaning.

At the time of the visit the school age area in the basement had some areas of chipping paint. Shed outside has chipping paint on the front doors.
At the time of the visit there was a wheel barrel outside of the fence blocking the gate to the parking lot.
At the time of the visit the refrigerator in the infant classroom was 45 degrees.

A review of medication found that one [REDACTED] was expired. Two prescriptions were expired for medications at the program.
At the time of the visit there was rust on the bathroom stalls in the men's bathroom. Stairs through out the program have visible dirt and cobwebs on them. Basement had a strong musty smell. Sink in the pre-k classroom upstairs was clogged.
A review of medication in the classrooms found one IHCP was expired and one was not signed by the child's parent.

At the time of the visit one staff member took one child inside to go to the bathroom leaving the other staff outside with 15 children more than 10 minutes.
At the time of the visit there were open crackers not in a sealed container.
At the time of the visit there was a sippy cup/bottle in the infant refrigerator not covered.
At the time of the visit the basket ball hoop and bikes were in the fall zone of the caterpillar. At the time of the visit the dome climber did not have the six foot fall zone on all sides. At the time of the visit one of the climbers did not have the six foot fall zone.
A review of medication consent forms found that one child had outdated forms and another child's forms did not have the parents signature.

Open food was not stored in a covered container in classrooms as well as the office.
Gate in downstairs preschool room, toddler classroom and the infant classroom were tippable and not secure. Surge protector in the toddler classroom was missing outlet covers on empty outlets and was accessible to the children. Equipment throughout classrooms was tippable and not secure. Visible dirt on the heaters in the preschool classroom downstairs. Chipping paint on the wall by the heater in the preschool room. At the time of the visit extra equipment in the infant classroom was stacked in the back behind the gate and was stacked in a way that could be a hazard.
Chipping paint was observed near the heater in the preschool downstairs. Chipping paint and rust was observed in the men's bathroom which the children use.
School Age children did not wash their hand prior to having snack in the church kitchen.
