Cleaning products (Clorox and Windex) and staff’s personal belongings (purses) were stored in an unlocked area of a classroom, acessible to children.
Various rest materials (mats) reflected tears along the edges which exposed the interior foam like material.
Documentation of a physical examination and record of a second MMR immunization were not available for review for a staff member hired on 10/10/2024.

A cleaning product (Comet powder cleaner) was stored underneath an unlocked sink cabinet of a restroom located inside a classroom.
A distance of at least two feet between each crib or cot, or there is a distance of at least three feet between children’s faces while resting or napping was not maintained in two classrooms as various mats and cribs were placed between one to seven inches apart.
This investigation inspection recorded no violations or advisories.
A {REDACTED} classroom attendance reflected arrival time for eleven children, however twelve children were present.
At the time of the visit, a staff's purse was placed on top of the children's cubby shelve, within reach of {REDACTED} age children.
Health and Safety
A {REDACTED} classroom did not reflect materials and equipment to visible, available and accessible to children at the time of the visit. Toys were stored out of reach and shelves located within the classroom space were ninety percent empty.
At the time of the visit, two refrigerators used to store children's meals reflected temperature readings of 46 and 51 degrees.

At the time of the visit, one staff record did not contain a current physical and three staff records did not contain evidence of [REDACTED]. The educator must provide documentation of a current physical examination at the time of each license renewal or more often if, in the Departments or licensees judgment, a medical condition requires a greater frequency.
In a [REDACTED] classroom the daily attendance did not reflect accurate daily count, five out of six children were signed in the attendance. In a [REDACTED] classroom six out of seven children were signed in the daily attendance. Daily attendance records indicating each childs attendance, including arrival and departure times must be documented.

A review of four staff records reflected the following. 1. Documentation of interview for four staff were not available for review. 2. Documentation of a second {REDACTED} date was not part of a staff's record file. 3. Documentation of a physical examination for two staff were not available for review.
A BRC review reflected a staff member with a pending BRC status since 2/22/2022 due to an expired fingerprint status and a second staff did not reflect a BRC suitability status under the BRC Navigator Portal.
At the time of the visit, an {REDACTED} milk formula stored inside the refrigerator was not labeled with the child's name. Three additional food containers were not labeled with the child's name. A child's pacifier was not labeled with the child's name.
A child's {REDACTED} medication ({REDACTED}) did not include an original container in which it was originally dispensed/original affixed label.
At the time of the visit, opened food packages were stored inside a cabinet, these were not properly stored in covered containers.
In a [REDACTED] classroom, an air freshener was placed on top of the counter, within the reach of children. In a [REDACTED] classroom, a staff purse, plastic bags and an air freshener were stored in an unlocked lower shelf cabinet which was accessible to children. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags and purses are locked or inaccessible to children.
