A soiled disposable diaper was not placed in a closed container that is lined with a leak-proof disposable lining.
1. Cleaning products (Comet, Disinfectant sprays, Windex, Clorox Bleach products) were stored inside two unlocked closet spaces, accessible to children. 2. A diaper rash ointment product was stored inside a storage bin inside a restroom, accessible to children. 3. Staff's personal belongings (three purses) were stored on the floor inside the kitchenette area, accessible to children as the door was not safely secured/locked. 4. Electrical cords longer than six inches from electrical devices were not safely arranged/secured in a preschool classroom and gross motor room. 5. A first aid bag containing emergency rescue medications (Two EpiPens) was stored within reach of children in the playground space.
This capacity increase visit inspection recorded no violations or advisories.

1. Cleaning products (liquid bleach and Fabuloso cleaner) were stored on the floor inside an unlocked closet on lower-level, adjacent to the gross motor room space area. 2. An over-the-counter medication bottle (Ibuprofen) was stored inside an open plastic bin on top of a shelf within reach of children in a {REDACTED} age classroom. 3. Electrical cords were not safely secured/arranged in three indoor licensed spaces (science room, {REDACTED} room and gross motor room). 4. A staff’s personal belonging (tote bag) was placed on top of a mini size fridge, within reach of {REDACTED} age children.

Two assistant staff members (unqualified) were observed caring for the supervision of seven children in a {REDACTED} classroom in absence of the classroom's designated qualified staff member.
1. Two children’s individual health care plans for an Epi Pen medication were outdated as of 6/30/2023 and 10/4/2023. 2. A child's medication consent form for over the counter medication (Zyrtec) was outdated was of 6/26/2023. 3. Medication consent form for a child's prescription medication (inhaler) was not available for review.

A review of two staff records reflected the following. 1. Bi-monthly observations and verification of references were not available for review. 2. Documentation of a program application/resume, verification of two references, documentation of an interview, program orientation, physical examination and MMR immunizations were not available for review.
A current staff schedule available for review, as the staff schedule posted in the administrators office was found to be outdated as of 9/30/2023.
1. A {REDACTED} classroom reflected peeling paint on a wall, behind a trash bin, which was approximately 18" in length by 2" width. 2. A {REDACTED} classroom reflected a rubber baseboard, half way detached from the wall, exposing broken plaster.
A review of the program's FY 2023 emergency evacuation observations reflected no documentation of a practice fire drill for the months of November 2023 and December 2023 available for review.
1. A child’s Epi Pen medication did not include the original packaging (box) in which they were originally dispensed and with their original labels affixed. 2. A child's over the counter medication (Zyrtec) was outdated as of 8/2023.
1. Adult size scissors (2) were stored inside a wooden bin located on an activity shelf, which was within reach of children in the {REDACTED} classroom. 2. An electrical cord longer than six inches from a standing lamp was not safely secured in the {REDACTED} classroom. 3. Items (Aquaphor healing ointments and buttpaste noting “Keep Out of Reach of Children” were stored inside a three tier plastic drawer inside a restroom, within reach of children.
1. In the {REDACTED} classroom, nine mats reflected to be torn along the edges, exposing the mats interior foam material. 2. Sleeping materials (mats) were not individually labeled.

A second child's record did not contain an updated physical examination (expired 6/7/2022). The child's Developmental History form was outdated as of 7/2/2021. The child's record did not contain {REDACTED} for 2021 and 2022.
A review of three staff records reflected the following. 1. Documentation of a physical examination were not available for review for two staff. 2. Documentation of {REDACTED} immunizations were not available for review for two staff. 3. Documentation of bi-monthly observations were not available for review for three staff.
A child’s Individual Health Care Plan for an {REDACTED} medication was valid until 7/1/2022.
