A BRC review conducted via the BRC Navigator Portal reflected a staff member's fingerprint status to have expired on 2/11/2024 and a second staff member's BRC status to have expired on June 17, 2024.
1. A child's individual health care plan for an Epi Pen medication was expired as of 2/8/2024. 2. Three individual health care plans for Epi Pen medications did not include parental consent.

1. An allergy list displayed in a classroom was not posted in a manner that protected the privacy of children. 2. A child's Individual Health Care Plan had been posted on the outer side of a cabinet door of a classroom which was not privacy protected. 3. The allergy list posted in the classrooms did not include medical information for a child requiring an {REDACTED} medication.
1. A staff's personal belonging (handbag) was stored inside an unlocked cabinet, accessible to children. 2. A significant amount of small size trash bags was stored underneath a table that was located in the lobby/front entrance. 3. Clorox wipe containers had been placed on the floor against a wall in the hallway, accessible to children.
A child's {REDACTED} medication lot number noted on the chamber did not match the lot number noted on the original medication packaging/box.

The program's outdoor solid rubber surface has detached from the concrete pathway, causing the rubber surface to lift around the edges which may result in a tripping hazard.
A significant amount of peeling paint was observed on a wall in an [REDACTED]room.
The following items were observed at the time of the visit; (1) Two staff's purses hung on a hook behind a door in a [REDACTED]classroom within the reach of [REDACTED]children. (2) A [REDACTED]product was stored within reach on an open shelf in a restroom used by children. (3) An electrical cord longer than six inches was accessible to children in a classroom.
