Documentation of an updated first aid emergency medical consent and release form, developmental history form and transportation plan was not available for in a child’s record file. 3. A child’s enrollment application was found to be incomplete as it did not include date and or signature from the parent. 4. An updated Children's Records Checklist was not available for review.
Documentation of a physical examination, MMR immunizations, and program orientation were not available for review in three staff record files.
A background check review reflected no BRC suitability status for a staff member working at the program. A BRC renewal application for a second staff member was not finalized, reflecting a "saved" status.
The program's Building Inspection recently expired on 9/25/2025. An updated Building Inspection was not available for review.
A review of the program's emergency practice fire drills did not reflect the exit route used. Documentation of the time of practice fire drills were not documented for the months of May 2025 through September 2025.
A child's prescription medication (Inhaler) was not stored in the original packaging in which the medication was dispensed in and with their original labels affixed.
1. An educator did not wash a child's hands with soap and running water following a diaper change. 2. An educator used hand sanitizer in lieu of liquid soap and running water to sanitize their own hands following a diaper change.

The children’s records checklist was outdated as it noted twelve children no longer enrolled in the program and did not reflect various children currently enrolled.
A Background Record Check review reflected two staff members with an expired background record check status as of 5/19/2024 and 6/1/2024.

A diapering changing mat reflected a large tear along the short edge, exposing the foam like material.

1. The outdoor window's metal trim located within the preschool playground space is detached from the wood beam, exposing the metal edge. 2. A corner of the vinyl siding covering located within the preschool playground space is unbuckling at the edges.

Documentation of the program's emergency evacuation observations were not provided for the months of July through December of 2021 and January through March 2022. The Emergency Evacuation observations did not include the exit route used.
Two cleaning spray bottle products (Restroom Cleaner) were stored underneath an unlocked sink cabinet which was accessible to children.
The Program Director reported being frequently in ratios due to staffing. No additional administrative personnel available to cover the desk while Program Director is in ratios. Review of written/required documentation reflected significant amounts of missing documents from staff and children's records.

Children's records reviewed on site reflected the following; (1) Two children's records did not include documentation of immunizations, enrollment forms, first aid and emergency consent forms and developmental history. (2) Four children's records did not include documentation of an updated physical and progress reports.
The allergy list posted in the classrooms dating 9/13/2021, included four children that no longer attended the program.
Staff records reviewed of nine new staff were missing a significant amount of documentation, including application/resume, references, interviews, physicals, {REDACTED}, staff information forms, program orientation.
At the time of the visit, a staff member was unaware of the total amount of children present, as they reported having seven children, however, the classroom daily attendance reflected arrival time for only five out of the nine children present in the room.
Thermometers verifying temperatures were not maintained in all refrigerators.
A Health Inspection conducted on 1/14/2021 reflected {REDACTED} expiration date of 5/14/2019 for the Certified Food Protection Manager.
The program does not have a person who is lead teacher qualified for the preschool age group served.
An individual health care plan for one child requiring an {REDACTED} medication dated 5/1/2019, for which expired 5/1/2020.
