A child’s individual health care plan dating 9/8/2025 did not contain a parent signature.
1. At the time of the visit, staff's personal belongings (purses) were hung behind a door, and additional purses hung on a second door in the kitchenette area, accessible to children. 2. At the time of the visit, an open concept toy storage shelf measuring approximately 6-7 feet in height was not safely secured, presenting a potential tipping hazard.

A child’s record reviewed did not include an updated Transportation Plan, Developmental History form, and an updated First Aid/Emergency Consent Form as of 8/30/2022.
Toxic Substances
1. Two staff records reviewed did not contain documentation of bi-monthly observations, Strong Start Essentials training Certificates, a physical examination, annual Professional Development trainings, PQR forms and an annual evaluation. 2. A staff's record did not contain documentation of a program orientation.
At the time of the visit, the arrival time was not properly documented for a child present in the group as the daily attendance reflected arrival time for twenty-two children, however twenty-three children were present.
The staff schedule posted on site, dated 9/2021 and did not reflect all staff currently working on the program.
A thermometer was not found in the refrigerator used to store food and milk products.
1. A child's Epi Pen reflected stored on site had expired on 4/2021. 2. A second child's Epi Pen medication did not include the original packaging/box in which medication was dispensed. 3. An over-the-counter medication (Benadryl tablets) did not include the original box in which they were dispensed.
Two children's Individual Health Care Plans did not contain written parental consent.
This enhanced monitoring visit inspection recorded no violations or advisories.
One child's medication (EPI PEN) for a chronic medical condition expired 4/2021 and did not include an affixed label/box.

In the technology room, two electrical cords were not properly arranged for an air conditioner and a television set (see attached).
The Building Inspection and Fire Inspection certificate had expired on 4/2/2019.

The emergency numbers were not easily visible to parents, educators and visitors, as it was posted on the side of a wooden shelf facing the corner of a wall.

Opportunities to foster independence and self help skills
(1) classroom and the second floor hallways had chipped paint from top to bottom. Three ceiling tiles in the technology room contained wet stains. One tile in the stairs hallway is buckling in half and should be immediately replaced (see attached).
