The licensor reviewed 3 of the children records. The following were observed: 1.Two of the children records were not up to date. The parents signed 4/15/25, 8/27/24. 2.One of the children records is not complete, the following are not filled out "Daily schedule"(page 5), "General permission" not specify where the child can go when off the premies, "Topical medication" is not specific (parent wrote "anything my child needs"). 3.Three of the children did not have updated physicals(the last physicals done on 9/23/24, 9/27/24, and 4/28/23. 4.One of the children records did not have the days and time of attendance (see page 2 on enrollment). 5.One one of the children records did not: A.specify where the child can go when off the premise B.Using what forms of transportations (see page 6 on enrollment packet). 6.One of the children records was not complete, the following were not filled out A.Regular medications B.Daily schedule C.General permissions when off the premises D.Topical medications/ointments not specify (see pages 4-6 on the enrollment).

The following were observed: 1.One of the children record is not updated because: A.The child did not have the correct information for allergies B.The child has a medical condition that may need the EPIPEN, medication is not listed C.The general permission for basic transportation is not filled out, if it doesn't apply, please note "N/A" C.The annual update is due, the last time that the parent signed was 10/1/23. D.The child record did not include child's picture or physical description. 2.One of the children record did not have child's picture or a physical description. 3.Three of the children records was not updated, the last time that the parent signed was 11/6/22, 2/29/25 and 1/18/23. 4.One of the children physical is dues as of 4/12/25.

The emergency plan is incomplete, the plan must have a written plan not limited to missing children, the evacuation of children from the program in the event of a fire, natural disaster, loss of power, heat or hot water or other emergency situation.
The educator has a play equipment with two slides that is not covered with an adequate depth of an impact-absorbing material. The playground equipment is from year 2020.

The licensor randomly observed 2 of the children records. The licensor observed one of the children records was not up to date when the child has [redacted] that was not listed on the enrollment form.
During the visit, the licensor observed that one of the children enrolled with a [redacted] did not have an individual health plan.
The educator did not have the 30 hours of training to renew this license and 10 of those hours must be in diverse learners. Educators in family child care working more than 10 hours per week must complete at least 10 hours of professional development per year.
The licensor observed the educator did not have progress reports for the children enrolled. The licensor observed 2 of the children records.
