The licensee must keep all equipment, materials, furnishings, toys, and games clean and in a safe, secure, and workable condition. The licensor observed: 1. The changing pad is not in good repair(ripped). 2. A plastic container (stand style) not secured.

The licensor reviewed four of the children's records. Even though the educator stated that two children are leaving the program, the educator must complete and update the children's records. The following was observed: 1. One of the children's records needed to be updated when the parent signed on 3/1/22. The new update was due on 3/1/23. 2. One of the children's records needs to be completed. The following pages need to be completed:4 5,6. 3. One of the children's records still needed an updated physical. The physical was due on 1/21/23. 4. One of the children's records did not have a physical. 5. One of the children's records had no physical description or picture of the child. 6. One of the children's records did not have the following pages complete: 4, 5, and 6
The educator needed a complete emergency plan. The following information needs to be included: a fire, natural disaster, loss of power, heat or hot water, or other emergencies.
The educator did not have the 10 hours of diverse learners to renew this license.
The licensor observed four of the children's records. The following was observed: 1. One child is not due for a progress report yet. 2. The progress reports were unavailable for two of the children.
The licensor observed the following unsafe for the children: 1. open trash can in the kitchen. 2. Peeling paint the side fence and the front entrance (railings)
The educator has four playpens. The educator needs two napping materials because the capacity is for six children.

At the time of the visit, the educator did not have evacuation logs in the year of 2021, and 2022.
