Four of six children's records reviewed did not include documentation of current physical exams. Three records had not been updated annually.

There was no current Health Care Plan on file for a child in care with an (REDACTED).
At the time of the visit, a person who identified themself as a therapist for the assistant went into the program to work with the assistant during childcare hours. The assistant is required to be working in the program at all times if there are more than 6 children present. Only EEC approved persons are allowed on the premises.

Current progress reports had not been completed and maintained in each child's record. Last reports on file were from 9/25.
The physical exam documentation for the assistant was not current and was not adequately completed and signed by a medical professional. Please submit current medical documentation regarding assistant's ability to work with children.
Please submit current emergency procedures for how evacuation procedures would take place for the assistant and all the children in care.
The diapering surface was ripped and in need of replacement. In addition, the surface was a port a crib changing station which is weight limited and too small for children other than infants.
There was no current medication authorization for an (REDACTED) medication prescribed for a child in care.
The educator was planning on serving apples to the children during lunch, however one child's record indicated the child was (REDACTED) on the enrollment form and medical. Although the educator stated the child no longer had the (REDACTED), the educator did not ensure that all records were current to reflect all health conditions.
Upon the licensor's arrival, the educator was not adequately supervising the children. The educator was upstairs on the first level while the children were all in the childcare space in the basement for at least 5 minutes. Children were attempting to let the licensor in the door until the educator came back downstairs.

Children's records and progress reports had not been updated and completed as required.
There were hazards in the outdoor play area: -The top of the fire pit was rusted. -There was a large pile of wood pieces, leaves and debris near the fire pit accessible. -There were several plastic climbers and slides too close to the fence without adequate protective ground cover and fall zones. -There was a rusty shovel leaning against the fence not secured. -There were loose wood pieces leaning against the deck.
There were two plastic slides on dirt. One had a crack and both were higher than 18 inches and did not have shock absorbing ground surface and fall zones. There was a retaining wall that was not adequately blocked off to prevent children from falling off it. There was a wood pit pile with sharp edges on some of the wood accessible in the play area.
There was a book shelf at the bottom of the stairs on the upstairs exit that was blocking part of the area.

There was a cloth pad on the diaper surface that was not impervious to liquids and was not washed and disinfected after each use.
Two children were not signed in upon arrival on the attendance. The provider did not document the assistant attendance when present.

Two of the area rugs in the classroom were in need of cleaning/replacement. There were some outdoor toys that needed to be cleaned.
Documentation of current rabies for the dogs was not available. Please submit.
The provider requested designating the fenced back yard as outdoor play area. The lattice around the bottom of the deck was not secured all around to prevent children from going under the area. There were several sticks and acorns in the area that needed to be removed. There were metal poles, a wood pile, and large stones that needed to be removed.
There was no documentation of required trainings for the assistant.
This follow-up visit inspection recorded no violations or advisories.

Educator did not have complete enrollment packets for two infants that were present during today’s visit.
The educator was out of her licensed ratios during today’s visit. The educator had seven children present of which none were school aged. Three were infants all under 15 months. The other four children ranged from 2-4 years old. The educator has an approved assistant who was present in the home but not functioning as her assistant during this visit. Discussed with the educator that’s because she has no school aged children enrolled she cannot have more than 6 children present on a daily basis and one of the infants must be 15 months if there is only one educator present.

Wood pieces, debris, tools, construction materials were accessible in area near entry door and near the driveway and needed to be removed. The retaining wall and under deck area was accessible. The back yard area that is not approved for child care was accessible. The wood post near the entry needed to be covered.
