Safe Sleep
All of the children's records were reviewed, three children did not have proof of lead testing.
In the bathroom, there were bottles of sunscreen, A&D ointment and diaper cream accessible. In the outdoor space, the trash can did not have a cover.
The (REDACTED) was observed to be sleeping in a pack and play with a folded blanket in between the fitted sheet and mattress. This did not allow for a flat firm surface.
The second exit on the first floor did not have an exit sign posted.
Provider had an unapproved person on the premises assisting in staffing the program.

Outdoor Playspace - Hazards
The basement space had indoor building materials which were being used, dust and paper covering the floor as provider was going to paint the space. This space was not being used for children at this time.
The outdoor space had small rocks accesible and a shovel. The provider does not currently have children in this space.

In the kitchen area a cheese grader in the cabinet was accessible to the children. There were several electrical outlets in the kitchen and living room area uncovered. In the bathroom, there was a box of plastic gloves and some plastic bags accessible in a drawer.
Indoor Hazards
The outdoor fenced in area is 630 square feet, which does not meet the required square footage for the outdoor space.
There was a slide set that is higher than 18 inches, and therefore, it needs the required ground surface, and the 6 feet in all directions from the perimeter of the equipment..
Water temperature was 135 degrees.

The stairs to the second floor were accessible from the living room, which is a licensed space.
The records were reviewed for all enrolled children. One child needed proof of current physical and immunization.

Educator was in compliance with ratios on the day of the visit. However, a review of the attendance records showed that the educator had too many children under the age of two years old from 4/25/2022-5/4/2022.

Safe Sleep
Care of young children - curriculum
The first aid kit was missing scissors and gauze rolls.
The educator washed her hands after diaper changing, but did not wash the infant's hands.
There was an outlet in the basement level playroom that was uncovered and accessible to children. The educator covered the outlet during the visit.
There was evidence of appropriate curriculum and routines for the older children, but not for the infants. The infants were confined to the porta crib the entire visit.
There was one surface that can be disinfected. The educator didn't use a second surface when she changed an infant's diaper during the visit.
The infants were in the porta cribs during the visit. The educator turned the infant over to her stomach so the infant could sleep. The infant did fall sleep. EEC Safe Sleep Regulations were reviewed. Safe Sleep Parent Letter was given to the provider. The provider read and signed the Safe Sleep Pledge. It was discussed that the educator needs to take the Safe Sleep Training.
The two infants were kept in the porta cribs during the entire visit.
The Educator's diapering table is located in the basement level. There is no sink available for hand washing in the basement level.

The Educator stated that her fine motor equipment was still boxed and not available. Please forward a picture of the fine motor materials.
Diapering gloves were accessible and need to be stored out of reach. The bungee cord placed on the basement door needs to be replaced with another safer lock. The television on the first floor level isn't secured and could pose a tipping hazard. The cords behind the television are accessible. Plastics and aluminium wrap were accessible in the upstairs kitchen draw.
