Out of four Children's Records reviewed, one lacked a Medication Consent Form for (REDACTED) and for a prescribed (REDACTED) for a child with a diagnosed (REDACTED).
Out of four Children's Records reviewed, the following was found: -Four lacked complete Individual Health Care Plans. For each child with a chronic medical condition that has been diagnosed by a licensed Health Care Practitioner, programs must maintain an individual health care plan (IHCP). The plan shall describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.

Indoor Space
The following were observed and accessible to children: -In the 2.9-4 year old classroom there was a two drawer plastic shelf that had a sleep sound machine on it with a cord longer than six inches. *Clorox Wipes and Swiffer Wipes were on the bottom purple shelf. *In the Kitchen drawer under the counter, there were Ziplock bags, AA batteries, Reynolds Foil Wrap, staples, pins, and stapler removers. *A broom and Swiffer Mop were in the Kitchen area. *Plastic chairs were stacked five high and were tippable, causing a potential safety hazard for children in the classroom. -In the 4 year old classroom Kitchen area in a drawer under the counter, there were plastic bags, Body Shop Aloe Body Butter, Cling Plastic Wrap, Reynolds Foil Wrap, and a screwdriver. *Wooden Chairs were stacked four tall and were tippable, causing a potential safety hazard to children.
Toxic Substances
In the downstairs hallway on the storage door, there was chipped paint.
