A review of BRCs showed that five staff members BRCs were expired for up to a month before being renewed.
Food prepared/served in safe/sanitary manner
Opportunities to foster independence and self help skills
Educators were observed serving children pasta salad using their gloved hands instead of a serving utensil. The pre-kindergarten children did not have utensils in which to eat their food, so they needed to use their hands.

Diapering and Toileting for children under 2.9
Two educators were observed changing diapers without keeping one hand on the child at all times.
During a health and safety inspection, the following was observed: 1. A cabinet door in the Twaddler A classroom is falling off the hinges. 2. A large oval rug in Preschool 2A was rolled up on the edge causing a tripping hazard.

An expired gallon of milk was observed in the Infant A classroom.

A health and safety inspection of the classrooms showed the following: * The chairs and a sofa in the "Preppers" library area were fraying and torn * There were holes in the side of the diaper changing table in the "Twaddler A" Classroom *The laminate on the top corner of the of a shelf in the Toddler C classroom was broken
Equipment is clean, safe, secured, and poses no hazard.

^Unsecured "kitchen" furniture was observed in Prekindergarten, Preschool 1, and Twaddler B. *A bookshelf in Twaddler B was unsecured.
A review of IHCPs showed [REDACTED] as a required medication, however there was no [REDACTED] at the program.

Staff orientation, training and meetings
When asked if any children in the classroom had emergency medication, the regularly assigned teacher had to check the classrooms placemats to see if there were any allergies.
Indoor Space
During health and safety inspection the following was found: edges of cubbies are coming off, a bookshelf was not secure, rust on the sides of refrigerators, diaper changing mat has hole in it, air freshener within reach of children, blue quiet area chairs have holes in them, vinyl baseboard peeling off and rug on play track is peeling up.
Attendance was not accurate in one classroom. Not all children were marked in on their electronic system.

The following was observed in teh Toddler B classroom: * A toilet brush was stored in the bathroom. * The cabinet door under the changing table has a broken lock therefore the children have access to diaper cream.
Review of 8 children's files found 2 with [REDACTED] and 1 with enrollment forms not dated.
Not all classrooms had an adequate amount of books in their reading areas.
Review of medication found two medications that had expired. One medication administration consent was expired and another medication didn't have a consent on file.
Not all items intended for a specific child were labeled.
A child was not observed washing their hands after toileting.
An IHCP was found with no parent signature/date.
Inspection of the indoor space found: a broken cabinet door in a diapering area, a bag of open trash on the floor in the Make Believe Boulevard bathroom, a staff purse within reach of children and broken molding on cubbies.
