A review of one staff file showed the educator was missing their required physical.
One educator was observed trying to supervise 13 children use the bathroom and wash hands while the other was getting snack ready in the kitchen. The children were spread out, not in the educators view and near the entrance.
A review of one child's medication consent provided by their doctor showed it was missing the parents' consent.

The program does not have a current staff record's checklist. A review of three staff files showed one was missing evidence of references.
The program director did not have a children's record checklist completed for review. A review of two children's files showed one was missing evidence of physical, immunizations and a lead test. The second file had an expired physical and first aid consent form.

A review of the children's record checklist showed the following: * 7 students were missing documentation of a current physical * 2 students were missing record of immunization * 1 student was missing the transportation plan.
One grouping of children had the master attendance but did not have an attendance for the current children in the group. The program director corrected by creating an attendance while the Licensor was present.
A small climber on the playground did not have the required 6 feet of fall zone, nor the required 9-inch depth of mulch.

A review of the attendance showed 12 children, however there were five children in the classroom. The program breaks up the grouping into separate groups to have learning experiences in different classrooms. Staff should each have an attendance to know exactly is in their group at all times. This was offered as technical assistance during the visit conducted on October 6, 2021.
A child’s emergency medication was not readily available to the child as he was in a different group while the medication was kept in a separate classroom. This was cited on the visit conduced on October 6, 2021.

At the time of the visit, the fire/carbon monoxide detectors in the gym area had all been removed from the ceiling and were placed on one of the gym windowsills.
At the time of the visit, the Licensing Specialist noted that following hazards in the new outdoor play space: 1) Three patio bricks were seen in the corner of the playground adjacent to the side of the building, which posed a tripping hazard 2) A large rain gutter downspout splash block was seen jutting out from the drainpipe which posed a tripping hazard.
Two outlets were missing the required covers.

This was cited on the prior visit conducted on October 6, 2021.
Though the program director stated fire drills are conducted monthly, a review of the evacuation log showed the last fire drill was conducted in February. The program was given technical assistance in this area during the visit conducted on October 6, 2021.
