Licensor reviewed Providers First Aid kit and found it contained all EEC required items except scissors. Please add scissors and make sure the traveling first aid kit has all required medical items at all times.
Provider has a diaper changing table with diaper pad. Licensor noted that Provider is not using a disposable layer each time. Please purchase a disposable layer and adjust diaper changing protocol.

Licensor noticed childrens belongings were in backpacks on the floor. Please provide a spot for children to store their belongings such as laundry baskets or cubbies and make sure children's items do not touch one another.
Providers back exit through basement doors was not cleared of ice/snow on day of visit. Please ensure all exits are usable and safe by clearing snow/ice and general obstructions daily.
Space (indoor / outdoor hazards) - R Apr 2025
Bathroom cabinet was observed not locked and contains cleaning supplies. Wooden toy shelf with bins in the basement was not secured to the wall and poses a tipping hazard. In the basement denim insulation, does not contain fiberglass, has been installed in the ceiling. In some sections of the ceiling the insulation is exposed. Wooden slats in place over some sections. Discussed covering exposed areas as well.
This monitoring visit inspection recorded no violations or advisories.
One child with a [REDACTED] located in Allergy list is not posted as required. Location of the EpiPen should also be included on the allergy list. Discussed putting a sign on the outside of the cabinet that says "EpiPen" and taking the EpiPen everywhere the child goes, including upstairs. An emergency action plan signed by the child's pediatrician is located in the child's file, however, the parents have not signed a medication consent form.

Attendance is tracked using a weekly sheet on a clipboard. Two children's arrival times were not filled in on the day of the visit. Educator filled in when pointed out by the licensor. Attendance must be accurate at any given point in the day.
The first aid kit appears complete with a CPR mouthguard, bandaids, gauze, gloves, etc. Ear thermometer with disposable covers. Emergency pages for the children were reviewed- 2 were outdated so parents will need to complete new ones.

The educator will need to email a copy of current CPR and FIrst aid certification to the licensor. It was not available at the time of visit.
Bathroom cabinet under the sink was not locked and contained a cleaning spray. Office desk and bureau in one of the bedrooms has small items such as jewelry, batteries, etc. which could pose a choking hazard. A large purse was accessible in the dining room Cabinet under the sink in the kitchen was not locked and contains various hazards.

Educator stated that the children sit on the kitchen floor to eat. High chairs and child sized table and chairs located in the basement. Discussed children eating downstairs and using this furniture to eat in.
Stairways
First Aid Kit (FCC)
Gate needed for the top of the stairs to the outdoor basement exit to prevent children access. Gate needed at the bottom of the deck stairs. Educator must email licensor photographs of the gates in place.
Large wooden pieces under the deck where the ride on toys are. Loose bricks and pieces of cement are tripping hazards in the backyard. Uncovered grill. A piece of the house siding is falling off in the backyard space.
Emergency contact phone list is complete and posted in the basement, but not on the first floor level.
