Random review of 8 children's files found one file with expired enrollment paperwork.
An educator covering a break in a classroom was not aware if any children in the classroom had emergency medication.
In two classrooms, educators did not ensure all infants hands were washed after diapering. One educator did not clean and disinfect in the correct order.
This investigation inspection recorded no violations or advisories.
A child's IHCP states [REDACTED] as a medication to use in emergency but the 2nd IHCP (EEC form) states to only use an epi pen. The IHCP also makes note of using an inhaler. Clarification needs to come from the doctor as to what medications need to be administered if an allergic reaction occurs. Another child's [REDACTED] states to administer the medication if the seizure lasts longer than 5 minutes but the educator stated it had to be administered if seizure lasted longer than 6 minutes.

In an infant classroom it was learned handwashing and food prep are being done in both sink basins. Educators did not clearly understand that one basin is for diapering/hand washing and the other is for food prep and the handles and spout must be cleaned and disinfected after diapering/before food prep.

Licensor had to remind an educator in an infant classroom to wash their hands after diapering. Licensor also had to remind an educator to clean and disinfect the diaper changing table after use.

Attendance in two classrooms was not accurate.
It was learned in some infant rooms, due to water not being hot enough to warm bottles, they bring hot water in plastic carafes when needed. This occurs during the day when infants are in the classroom.
Educator in one infant room stated they are washing hands after diapering at classroom counter sink (halfway across room) and not in the bathroom immediately behind the diapering table.
Educators in one classroom could not adequately clean/disinfect due to their disinfectant bottle being taken by another staff for refill.

Two educators being used as teacher qualified staff did not have completed evidence of qualifications on file. One did not have a work experience form on file and the other had an inaccurate form on file.
An infant was observed laying on a boppy drinking from their bottle. Three educators were in the room and only 1 of the educators was caring for another infant.
During health and safety inspection the following was found: not all area rugs were clean, a bathroom did not have toilet paper, some tables had sticky residue on them and windows were dirty

Review of 8 children's files found the following: 6 files contained enrollment forms that were either outdated or not signed/dated, 4 files had no current physical and immunizaitons, 4 had no evidence of lead screening and 2 had no transportation plan.
During review of staff meeting documentation it could not be determined if a minimum of 2 hours is held monthly.
Attendance wsa not accurate in a toddler classroom. Nine children were in attendance and only 7 were marked on the roster.
A [REDACTED] expired as of 1/2019.
Child was observed toileting and then returning to an activity without washing hands.
One child's IHCP was not signed/dated by the parent. Two individualized health care plans stated [REDACTED] as part of the plan and there was no [REDACTED] on site for these children.
Health and safety inspection found the following within reach of children and potentially hazardous: phone chord hanging low within, cup of tea on a low shelf, dirty toilet bases throughout center, storage in bathroom used by children not secured, brooms with excessive dirt, door handle to classroom not working properly, toilet paper cover broken, slow sink drain, carpets not vacuumed, blue couches in hallway have torn edges and wooden railroad ties on the playground have nails sticking out.
Peeling paint found in multiple areas accessible to children throughout the center.
