A review of the staff meeting log showed there was no documentation of staff meetings being held January-March
Through a review of five staff files, the following was found: 1. One file was missing a staff form 2. One staff file was missing proof of orientation and an expired physical. 3. One staff file had an incomplete staff form, no record of physical,[REDACTED]First Aid, or completion of the required Strong Start Essentials trainings.
The nap mats in the infant room were torn.
The Monkey and Tiger classrooms lack a variety of accessible materials such as dramatic play, art, and sensory.
Through a review of 10 children's files, 5 students had expired Developmental histories,
A review of the fire drill log showed the program is not conducting monthly fire drills.
A review of 6 IHCPs showed that 5 were missing the required training information, 1 was missing potential side effects of treatment and consequences if treatment is not given, and 1 had two chronic conditions listed, but only provided the plan for one of the conditions (there should be two separate IHCPs).
1. Shaving cream was observed stored in a closet and through an interview with staff, it was learned the children use it for sensory play. 2. A student was observed playing with their drawstring bag by wrapping the strings around their body and shoulder area which is a strangulation hazard. 3. Chairs were observed stacked too high in the Monkey toddler room 4. Diaper cream was observed stored in an unlocked cabinet and accessible to children.

Lifting tiles causing a tripping hazard were observed on the small playground in the front of the center with the "climbing tree".
A "Swiffer" wet jet mop was observed stored in the infant bathroom within reach of children.
A review of IHCP and emergency medication showed one students prescription, medication consent and IHCP were expired. Program was aware and working on obtaining current forms.

A review of the staff records checklist showed two infant staff did not have the required proof of [REDACATED] vaccinations and four staff (two being infant teachers) have not completed the required Essentials Training.
An educator was observed feeding an infant a bottle while the infant was sitting in a bouncy seat.
A stained ceiling tile was observed in the "Ladybug" preschool room.
The building inspection expired in February 2023.
The "tiles" lining the ground of the infant playground are lifted and separated, causing a tripping hazard.
The playground mulch is not the required 9-inch depth around the climbers and swings.
Through an interview with the Assistant Director, it was learned that fire drills are not being conducted on a monthly basis.
* A "Swiffer" wet jet was observed stored withing reach of children in the "Lady Bugs" preschool. * Diaper cream was observed stored in an unlocked area beneath the changing table in the "Sunflower" infant room. Staff stated they would move it out of reach immediately.
An educator in the infant program was observed changing a child's diaper and neglecting to wash the child's hand upon completion.
