Staff effectively communicate with each child
The following was observed: 1. In the "Ladybugs" classroom: the corner of a mat was rolled up causing a tripping hazard 2. In the "Fireflies" class: the cushion on the library couch had vinyl peeling off it.
A review of 4 IHCPs showed two were missing side effects, of medication, consequences if treatment is not given and training info. One was missing training info.
Dried black beans and small wooden disks were observed in the toddler sensory tables which presents a choking hazard.
An educator was observed quickly rinsing off a child's hands after a diaper change without the use of soap or proper rubbing of the hands.
The classrooms "Bumblebees" and "Fireflies" were missing an evacuation map:

A support educator in the "Ladybugs" classroom was visibly frustrated with the children: Not smiling or acting warm toward the children and moving them about physically. Upon learning this, the program removed them from the classroom and sent them home for the day.
A support educator in the "Ladybugs" classroom was observed to be visibly frustrated with the toddlers, using phrases such as "all done" "oh my God" and "Oh Jesus" around the children rather than positive guidance and redirection. Upon learning this, the program removed them from the classroom and sent them home for the day.
Several areas of the Inchworms preschool classroom had chipped paint. The program director stated painting is scheduled.

A review of the infant attendance showed 6 children signed in however there were 7 children in the classroom.
The following was noted during a health and safety inspection of the playground: * The concrete section of the playground is cracked and lifting in multiple areas causing a tripping hazard. * The pour and play under the preschool climber is cracking in two areas.
This renewal - monitoring visit inspection recorded no violations or advisories.

Indoor Space - free of hazards
Unlike all other classrooms in the program, this classroom does not have a phone. The [REDACTED] reported that the program's plan in a true emergency would for the classroom teacher to contact emergency services via their classroom phone. At the time of the visit, there was not a clear plan in place for this classroom of what the teacher who will be assigned to this classroom should do if they needed to call for emergency services other than shout for help.
Although two sectional of a cubby unit were connected together, the equipment was not not secured to the wall at the time of the visit, and therefore it could not be determined if it was tip-able. Discussed with the [REDACTED] securing the cubby units to the wall as the [REDACTED] reported the cubbies in all of the classrooms are secured to the wall.

Through a review of three children’s files, one file did not have evidence of a transporation plan of how Child A regularly arrives and departs the program daily.

An [REDACTED] Educator was seen caring for 16 pre-k children alone, while the Educator assigned to this classroom was helping a child in the bathroom. The bathroom area is not within the pre-k classroom.

While observing a diaper change it was noted the educator did not follow the two-step process of cleaning the changing table before disinfecting it.
A dust pan and broom were observed stored on the floor in the Butterflies bathroom.
The attendance in the infant/toddler classroom was not accurate as there were seven children present and six children signed in.
At the time of the visit, the following hazards were seen within the indoor space: 1) A couple of floor tiles in the preschool classroom were chipped, creating a potential tripping hazard. 2) Four wooden play kitchen items in the preschool classroom were found unsecured. 3) Disposable gloves and a plunger was found accessible to children in the bathroom.
An Educator in the preschool classroom whom reported that she was a substitute and not the assigned Educator for the classroom did not readily know the allergen for a child attending this classroom who had food allergy.
