The following was observed and accessible to children: In the PRESCHOOL classroom, there was a ripped seam in blue carpeting, and the small oval alphabet rug was not secured and a potential tripping hazard.

A review of all four staff records indicated the following: 1) One did not have evidence of a staff information form, 2) one did not have evidence of a second reference check, and 3) one did not have evidence of a second MMR immunization.

A review of six children's records indicated that one did not have evidence of a current physical examination, immunizations or lead screening results. It was also identified during the visit that the program did not maintain a records checklist of all children enrolled.
A review of the injury/incident log indicated that behavioral incidents were not documented.
It was observed in the PRESCHOOL classroom that an Educator's personal items were stored on top of the stack of rest cots.
A review of four IHCPs indicated that two did not identify which staff are designated and trained to administer the medication.
A review of three medication consent forms indicated that two were incomplete to include if the medication was taken before, what the side effects of the medication are, dates and times to give the medication, and directions for storage.

The following was observed in the PRESCHOOL classroom and accessible to children: 1) The plastic tool bench was unsecured and tippable. 2) There was an electric floor fan against the wall. Electric fans, if used, must not be accessible to children. 3) There was a sound machine electrical cord unsecured.
