It was observed that there was chipped paint under the chalkboard in PRESCHOOL 2.
The following was observed and accessible to children: There was rust on the return air vent in TODDLER 2, the bookshelf was unsecured and tippable in PREK 2, and there was a cracked floor tile and rust on the return air vent in PRESCHOOL 2 children's bathroom.
A review of four children's records indicated that one did not have evidence of a current physical examination.

A review of five staff records indicated the following: One did not have evidence of a program orientation completed, one did not have evidence of a current physical examination, one did not have evidence of immunizations, one did not have evidence of work experience for their respective position, and one did not have evidence of documentation to meet the Minimum Hiring Requirements.
A review of the evacuation drill log indicated that a drill was not conducted in November. The educator must hold practice evacuation drills with all groups of children and all educators of the approved space at least monthly.
A review of four IHCPs indicated that two did not identify which staff were designated and trained to administer medication in case of an emergency and all four did not have evidence of a written parental consent to administer medication.

The preschool classrooms eat lunch in the cafeteria. The preschool educators did not bring their attendance with them to the cafeteria. The licensee must maintain: 1. daily attendance records indicating each child’s attendance, including arrival and departure times; 2. a method of knowing exactly who is present on the premises at any given point in the day.

During the visit, five children's files were reviewed. One file did not include a current physical. Two files did not include evidence of a lead test.
In the PreK 1, Preschool 2, and Toddler classrooms, plastic baggies were located in drawers that were accessible to the children. A paper cutter was accessible to the children in the Kindergarten 2 classroom.
The September 2022 evacuation drill was not documented as required by EEC. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
