Licensor randomly reviewed 4 of the children's records. Of these, 1 was complete and up to date, 1 had an out of date physical, and the other 2 had no dates with the signatures in the enrollment packet. Please review all of the children's records and ensure they are all complete and up to date.

There are a couple of children who do not have up to date progress reports.
Licensor inspected all of the licensed, indoor spae during the monitoring visit, as well as the outdoor playspace (fenced in front yard). The following hazards were observed: ** KITCHEN/DININGROOM/LIVINGROOM **- drawers in hutch in diningroom had scissors, small chokable items that need to be removed or made inaccessible ** MUDROOM/ENTRYWAY **- no hazards ** PLAYROOM **- no hazards ** BATHROOM **- no hazards ** OUTSIDE **- hammock needs to be made inaccessible to the children.
Diapering is done on the rug in the bathroom, which Educator stated she changes out and washes often. Discussed that according to regulations, diapering must be done on a surface that can be easily disinfected after each use. A disposable layer must also be used in between the child and the diapering surface.
Licensor inspected all of the licensed, indoor space as well as the outdoor playspace. No hazards were observed in any of the licensed rooms inside the home. Outside, there is a large play set with no fall zone. This is a new playset that Educator just used to replace the old playset. Discussed that a full fall zone needs to be added before the children can use this set.
Educator stated she has completed multiple training this past year. However, Educator did not have any printed training certificates, so Licensor was not able to verify this.

Licensor randomly reviewed 5 of the children's records. Of these, 1 was complete and up to date, and the other 4 had out of date signatures for the enrollment packet. There were also 2 records that in addition to having out of date signatures, also did not have physicals. The only medical information in these 2 records were the immunization histories. Please review all of the children's records and ensure they are all complete and up to date.

Indoor Hazards
Licensor inspected all of the licensed rooms during the renewal visit. The following hazards were observed: ** KITCHEN/DININGROOM/LIVINGROOM - drawer to the right of the fridge has a child safety tab that is not working and there are hazards inside, the same is true for the cabinet under the sink ** MUDROOM/ENTRYWAY - no hazards ** PLAYROOM - tall storage unit is a tipping hazard ** BATHROOM - no hazards

Outdoor Playspace - Hazards
The children's progress reports were most recently completed in January, so they are all out of date.
The only hazard in the outdoor space was that the fire pit was not covered.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: ** KITCHEN/DININGROOM/LIVINGROOM - 2 drawers with hazards inside, 1 has childproof latch that isn’t catching ** MUDROOM/ENTRYWAY - no hazards ** PLAYROOM - no hazards ** BATHROOM - no hazards ** OUTSIDE - an above ground temporary pool, which is only 32 inches tall is accessible to the children. There are a multitude of various items, including car parts, machinery, etc. in the driveway and on the side of the home, which the children have access to when they are entering and leaving the program. These items need to be made inaccessible.

CPR/First Aid just expired the end of 07/2022. Educator needs to complete her recertification immediately.
Educator's in-laws are visiting and staying in a camper in the side yard. They have reportedly been there for approximately 1 month so far. Licensor discussed the need to submit a Change of Household transaction identifying these long term guests, so that EEC can complete the BRC process.
Educator reported that all of the children's progress reports are overdue. Please ensure that all of these reports are completed as soon as possible.
