Attendance Recording and Group Sizes/Ratios
Two children's records had IHCP'S that did not have current doctors authorization.
Missing program orientation, bi-monthly observations, evidence of qualifications. Another file missing program orientation, observations, evidence of qualifications, documentation of interview, First and CPR certification.
The staff schedule posted was not current.
During the visit the program had one Program Administrator and two assistants with 31 children.
Emergency medications readily available
Two children with medical conditions had expired Individualized Healthcare Plans.
Individual Health Care Plan (IHCP) - R Apr 2025
Emergency medications were locked in the cabinet that could only be opened with a key. A child prescribed an emergency medication (albuterol) did not have the medication on site.

Missing medication consent forms for two epi-pens.
No thermometer in the fridge.

Both staff files are missing required items on the staff orientation.
Evacuation procedure was missing at two exits in the cafeteria.
There are children which require an Epi-Pen, the Epi-pen was not on site for the children. The site coordinator reported that the children are new and the families have been notified that the Epi-Pen needs to be at the program.
