Evacuation procedures posted at each exit
The evacuation procedures did not reflect the procedure for evacuating non-ambulatory children.
Supervision of children
While observing the infant room teacher changed a child's diaper, they partially closed the door behind them to prevent the other two infants from going into the bathroom. The Teacher could see and hear the infants through the glass window in the door, but the infants could not see their teacher and began fussing and trying to get into the bathroom. While observing the preschool classroom, the Program Director and the Licensor observed the mirrors hung to provide full site of the classroom, while the Teacher assist children in the bathroom or has to wash their hands. The Licensor pointed out a blind spot the mirrors do not cover.
Supervision of Children
While the Educator was helping children in the bathroom, a child was jumping off a chair, out of sight from the educator. The educator walked from the bathroom, to the classroom while a child was at the bathroom out of sight from the educator. An infant was sitting alone feeding themselves a bottle. The infant teacher left the classroom to warm up bottles in the kitchen sink and had their back turned to the infants. A toddler fell out of their chair when upset, landing on their head and face, the licensor told the educator the child hit their head, the educator did not inspect the child to ensure they did not get hurt, and they did not apply ice to the child’s head.

The preschool classroom located on the second floor, did not have window guards on four windows. Two toddler was sitting at the lunch table in the green chair, the child’s feet did not reach the floor, they leaned back when upset and fell onto the floor face first.

This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
Infection Control/Hand Washing - R Apr 2025
There was chipped paint in toddler preschool room to the right of sliding glass door and under the window, next to the small table. There was chipped paint in the preschool classroom in the block area above the plexiglass.
An educator was observed wiping a child’s nose with bare hands then failed to wash their hands. An educator changed a child’s diaper, then left the bathroom and came to the sink in the classroom and washed the child’s hands where they food prep, and infant bottles are heated.
