PQ# 9801061 Expired 10/27/24
Licensor randomly reviewed 2 of the children's records. 1 of these records had no record of lead testing, and the other record had out of date signatures in the enrollment packet, an out of date physical and immunization record.
Educator has created her own handbook, which is very detailed. However, the section for the emergency plan is missing some information.
All of the required items were observed in the first aid kit. However, some of the children’s emergency cards were out of date.
Educator needs to add escape routes and where would she go if the area needed to be evacuated to her Emergency Preparedness Plan.
Educator stores the napping materials together. Discussed that in order to avoid cross contamination, regulations states the napping materials need to be stored so they are not touching each other.
There is a child currently enrolled with an allergy, who is prescribed [REDACTED] This information was not posted in the program. Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to [REDACTED]medications that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality.
Licensor reviewed 3 of the children's records. Of these, 2 were complete and up to date. The third child has a [REDACTED] and is prescribed [REDACTED], but there is no paperwork in the child's record verifying any of this information.

Emergency Evacuation Logs
All of the required items were observed in the first aid kit, except an instant cold pack. Educator also needs to add the children's emergency contact information to the kit.
Educator's Evacuation log was up to date. The last fire drill was conducted on 12/26/23. However, the exit used during the drill was the same every month last year, and the time it took to complete the drill was not documented for any of the fire drills.
Licensor inspected all of the indoor, licensed space during the progress visit, as well as the outdoor playspace. The only hazard that was observed was in the playroom. There were cleaning bottles on the counter.
Outdoor Playspace - Hazards/Porches and Decks
Educator has installed fencing that is now 4 ft high and is stable, which meets regulations. However, there is no gate in the fencing. Also, there is a gap between the house and the fence that is an entrapment hazard.
