Emergency cards were not current in the First aid kit used for transporting children off premises.
The emergency preparedness plan did not include procedures when children are transported off premises on a regular basis. Including a plan for appropriate supervision of children in public spaces; and how the educator confirms the appropriateness and safety of each off-site facility prior to each use.
The educator had transported children off premises in a vehicle with a rejected inspection certificate. There had been no adequate seat belt cutter in the vehicle during transportation of children on the day of the visit.
The educator was not adequately notifying parents prior to taking children off the premises each day for each activity. The educator did not ensure that each child was carrying with them the name, address and telephone number of the educator whenever off premises in the care of the program.
The educator did not have a written plan for transportation including how children are transported to and from the program to off-site activities and for the safety and supervision of the children during transport.
The area of the basement bathroom was accessible were there were exercise equipment and unsecured items accessible. There was no secured gate at the bottom of the stairs accessible on the first level. The corners of the stone table in the living room were not covered. The tall lamp in the living room was not secured. The wooden laundry doors were a pinch hazard and were not secured.
The educator did not exercise appropriate supervision of the children in her care. Upon the licensor's arrival at the home, five of the children, all under the age of 5, were sitting in the outdoor play area eating while the educator was inside the home with the 6 month old infant for several minutes before returning outside with the infant. In addition, the educator stated the infant had been sleeping alone inside the home while she and the other children had been outside.

The ground surface under and around the swings, slide and Monkey Jumper was hard packed and rocky. Fall zones did not have required area of 6 feet of protective ground cover under and around the swings, slides and monkey jumper in the outdoor area.
Stairways
There was no gate at the top of the deck stairs. Child care uses deck area for children under 3 years.
