A review of two Staff Records at the time of the visit indicated the following: -One record did not contain evidence of a MMR immunity and reference checks prior to employment. -The second record did not contain evidence of immunity to MMR or a current PQR.

Building Inspection and Fire Inspection - R Apr 2025
The program is housed in a public school. The building inspection is not current and expired on 4/30/24. The most recent fire inspection certificate on file with EEC is 5/31/23. During the visit, the Licensee stated the school is aware of this and is working to make repairs in order to obtain certificates from the town.
The program did not have evidence of a fire/evacuation drill for the month of December 2024.
A review of three Children's Records indicated the following: -One child's record did not contain an Individual Health Care Plan for a diagnosed medical condition. -One child's IHCP was incomplete and did not have a parent/guardian signature.
Indoor Space
A review of medication stored onsite indicated the following: -One child's prescribed medication was not onsite. The program did have documentation that they had made attempts to obtain this medication from the child's parent. However, any child prescribed emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors must be onsite in the event of a potential medical emergency.

A walk through of the facilities indicated that in room 504, there was a large wooden bookshelf that was not secure and was tippable.
Upon a review of three Children's Records, the following was identified: -One child's record did not contain a Medication Consent Form for a prescribed potentially life saving medication.
Staff BRC - R Sep 2025
The program recently changed from a Funded program to an EEC licensed program. In the BRC Navigator system, eight staff were listed under the previous program and need a suitability transfer. All staff must have suitable BRC's in the EEC BRC Navigator system under this specific program (P-259198).
