A walk through of the facilities indicated the following: -In the Toddler I class, the children's nap mats were not a minimum of 2 feet apart.
The Staff Schedule was not posted in the program at the time of the visit.

-During the visit, the T-1 classroom had 10 children, with mixed infants and toddlers. This program is not currently licensed for a mixed age group of Infants/Toddlers. In addition, the maximum group size for a mixed Infant/Toddler grouping is 9. -The Director stated the program was using BrightWheel app to record arrival and departure times of children's attendance.
Upon a walk through of the facilities, the following was identified: Infant Room: -There is peeling paint on multiple walls in the classroom. -The multi-colored foam climbing pad on the floor is frayed and torn and in need of repair and/or replacement. -In the crib room, one crib was blocking an egress exit. This crib must be moved so as to not block an exit in the event of a potential emergency. Preschool Room: -The refrigerator must have a child safety lock so it is inaccessible to children. -There are multiple walls in the classroom with chipped and peeling paint.
Upon a review of Medication Consent Forms for enrolled children at the program with a diagnosed medical condition requiring medication, the following was identified: -One Child's Record did not include a current Medication Consent Form for prescribed (REDACTED).
Upon a review of four Children's Records at the program, one child with a diagnosed (REDACTED) did not have an Individual Health Care Plan (IHCP). A second child diagnosed with (REDACTED) did not have a complete and current IHCP.
